CMS says readmissions pilot program is working

The chief medical officer of the CMS Barry Straube, MD, said in a recent Associated Press article that the hospitals participating in the CMS-sponsored Care Transitions Program are reporting that the number of readmissions at their facilities is declining.

The news comes eight months after CMS announced the 14 communities participating in the pilot program, which aims to “improve health care processes so that patients, their caregivers, and their entire team of providers have what they need to keep patients from returning to the hospital for ongoing care needs.”
