Standing order labs

How do your facilities handle registration of “standing order labs”? I am looking for a process that will streamline the patients rather than having to stop at registration each time.

  1. Our standing order patients report directly to the lab for their standing orders. There are problems with this as these patients then want to go directly to the lab for any labwork, but it does make the patients happy. I know some facilities use a recurring account number for standing order lab patients, we however do not due to issues with Medicare overlaps.

  2. At our facility, we require a new registration with each visit to the hospital so there really is no way to make standing labs a recurring visit.

  3. We have a lab policy for this. They are permitted as long as they are valid, documented, medically ncessary, and monitored for appropriateness.
    Must have a beginning and ending date(not more than six months)
    Frequency & duration statement of noted on the order and must match the appropriateness of the test/service ordered.
    We utilize electronic records, so the order is pulled and reviewed for appropriateness for each test.
    Recurring can be used for lab such as coumadin lab checks. These accounts are discharged if the patient comes in for admit or observation. Physical, occupational, chemo orders are also types. Recurring is only used when necessary. Physicians are contacted if any of the requirements are not met.

  4. Do you have the patient stop each time to register since you are not doing reoccurring?

  5. The standing orders are recurring. We only allow RCR’s for certain services, this being one of them. Sorry I wasn’t clear

  6. We also do not create recurring accounts due to overlaps. We give the Patients that come in weekly/montly for labs a card with their med record # and a phone # to call prior to coming in for services. We get them reg’s before they get here. They go to the check in desk sign and go………..

  7. How do you address samde day add-on labs or imaging services? Are these billed separately on rolled into one? You said you discharge on admission. have you tried to use the occurrence span code 74 to indicate leave of absense instead?

  8. We use Recurring accounts that are discharged every 30 to 60 days..

We use a monthly recurring account for standing labs, PT, OT, Speech, Psych, etc. The patients must show at Patient Access on or about the first of every month to register and receive a new card. The office in each area is responsible to collect the copay, if due. This process needs to be streamlined but the card works great as a patient satisfaction tool.

  10. Would you share your policy on how the recurring lab accounts are handled? I’d love a copy to my email address? We are currently in the process of implementing recurring accounts for standing orders.

    once monthly as series,if the order read weekly, bi-weekly or every 3 weeks. The pt will register the first visit and told to go directly to lab thereafter. If any changes occur during the month the lab will call and send pt back to reregister.Labs keeps a copy of orders and check computer for last visit. Once a month standing orders are registered as outpts.

  13. Wentwoth Douglass, Standing orders are registered once a month through automation of our system, if the order reads atleast once a mopnth. The pt will register the first visit and to lab thereafter. We have a special consent for standing orders which is good for a year.If any changes occur during the month the lab will call registration to update information. The lab keeps a copy of orders. I am currently struggling with ABN’s they need to be ran every 90 days on standing orders. Can anyone share what their process is?

  14. How long do recurring radiology/laboratory orders last BEFORE new order is required?

    Our radiology department is asking if a recurring order can last 1 year before a new one is required. Do you know about that or could direct me to where I could obtain this information?
