A list not to make

It was good to see that no healthcare facilities had the dubious distinction of making the newly updated OSHA Severe Violator Enforcement Program (SVEP) log [1].

According to OSHA CPL 02-00-149 [2], businesses make the SVEP list when an inspection finds one or more of four criteria below:

  1. Fatality/Catastrophe Criterion.
  2. Non-Fatality/Catastrophe Criterion Related to High-Emphasis Hazards.
  3. Non-Fatality/Catastrophe Criterion for Hazards Due to the Potential Release of a Highly Hazardous Chemical (Process Safety Management).
  4. Egregious Criterion.
