Contest entry: Note-taking form

notetakingformHappy Free Form Friday! Here’s another great form that came in for The Greeley Medical Staff Institute Symposium contest.

Tommie Cooper, CPMSM, director of medical staff services at Sky Ridge Medical Center, developed this clever note-taking form for physicians. To prevent them from inadvertently walking away with documents that shouldn’t leave the meeting room, Tommie prints them on colored paper (a different color for every month) so that everyone is aware of what documents are coming and going.

Medical Staff Coordinator Shirley Prihoda, CPCS, of Brazosport Regional Health System submitted this tool.

Shirley says, “This was created to give the Credentials Committee, MEC and the Board a quick summary of initial and reappointments to have a more concise view of the applicant.”

You can download the Medical Staff Summary Template (Word doc). Remember to select “view” and “print layout” to display the actual document.

Kim Everett, medical staff coordinator at the Good Samaritan Hospital in Vincennes, Indiana, uses a great, comprehensive application flowsheet while credentialing physicians and allied health personnel during the application process.

Kim says,

“It has been a valuable instrument as the credentialing staff process and verify information contained in the application. It also provides coworkers with immediate information regarding the status of the application, as we can quickly see references that remain outstanding, any board certification or licensure issues, along with  unresolved issues or potential ‘red flags.’

The application flowsheet is also used as a quick reference when presenting the application to the Credentials and Audit and MEC committees, as they consider the applicant for membership and privileges.

Thank you to all the readers for their great work and entries submitted for the August giveaway for two free seats at the Greeley Medical Staff Institute Symposium (November 8-9, Naples, Fl).

July’s giveaway went to Nancy Bertling, RN, MBA, quality manager of TOPS Surgical Specialty Hospital in Houston, TX for submitting a physician report card for quality and re-credentialing.

This time, August’s giveaway goes to Lana Heavilin, RN, a medical staff office coordinator in the quality department of Holland Hospital, in MI. Lana submitted tools for physician reappointment. She writes:

“We were finding when a physician came up for reappointment, and he had procedures with number [of] requirements to maintain to privilege, he/she might be surprised to find they didn’t meet the numbers, and has not time to rectify the problem.

So, every six months I run a report out of our software program (Midas) and give each provider a report of the privileges they have requested, the number needed to maintain the privilege and the actual number they have done. This way there are no surprises at reappointment time!”

Free Form Friday returns: ED call compensation white paper

We received a terrific response to the free forms we posted on the Credentialing Resource Center blog every Friday in January. Now we have another great form to share with you–for free!

Authored by Rick Sheff, MD, CMSL, chair and executive director with The Greeley Company, this white paper provides insights for both medical staffs and physicians to approach ED call coverage and compensation in the fairest way-as a negotiation based on interests and principles.

“Physicians and hospitals are struggling to determine which physicians should be paid for call and how much they should be paid, which, in essence, is a negotiation…The problem is that most physicians have not been trained in negotiation,” says Sheff.
