Contest entry: Initial appointment checklists

Want sample forms? Here are medical and dental initial appointment checklists that come from Anita Vega at Sonoma County Indian Health Project. Anita writes:

“[W]e use these from beginning to end of the process. This keeps current track of the documents sent out, received, and those still needed. I use a pencil to write in notes. When completed, this document becomes part of the packet presented to our board showing that all documents are in the file and dates received. This also gives them an order to look at the document.  At this time, I am creating a check list for each different department.  Thus making it easier to know what is needed.”
